The Importance of Strengthening Primary Health Care for Universal Health Coverage

The G20 summit hosted in New Delhi by Prime Minister Narendra Modi was an opportunity for discussions on key policy matters. India highlighted its commitment to universal health coverage and launched the sixth edition of One Earth and One Health Advantage Healthcare India 2023. The efficient functioning of primary health care centers is essential for achieving universal health coverage.

Primary health care centers (PHCs) are the foundation of the healthcare system, providing basic services to the community. The OECD report emphasizes the vital role of primary health care in delivering effective health education, prevention interventions, and improving equity of health outcomes. In India, various states have implemented models of primary care, and the central government has introduced health and wellness centers under the Ayushman Bharat scheme.

However, the proliferation of private hospitals and clinics has led to a shift in focus from primary health care to private healthcare. This trend is evident in urban areas, but it is also starting to affect rural regions. To address this issue, there is a need for awareness, investment, and rethinking.

Firstly, awareness is crucial in reinstating the importance of an efficient primary health care structure. Privatization and access to specialty clinics have led to a disregard for primary care. There needs to be a shift in mindset, emphasizing the importance of primary health care for minor ailments and the concept of family medicine.

Secondly, investment, especially central investment, is necessary to ensure uniformity in primary health care facilities across states. Currently, the responsibility of investment lies with the states, leading to disparities in the quality of care. For universal health coverage to be achieved, there must be a mandatory and enthusiastic investment in the primary health structure.

Lastly, there is a need to reassess the primary health care needs of the community. The current system follows guidelines from 1946, which may not be relevant to the current healthcare challenges. There should be a policy-level reassessment of primary health care needs to establish an efficient and relevant approach.

By focusing on awareness, investment, and rethinking, universal health coverage can be achieved. This means providing healthcare facilities and addressing health challenges in even the most remote locations.
